Where it usually breaks

And how to keep it from happening.

  • A check has an observation but remains Pending. Resolve its result before requesting review.
  • A decision is attempted on a record that is no longer in Review. Inspect the current work before requesting another action.
  • Repeated disposition creates confusion about the same failed check. Compare the existing outcome and linked issue before proceeding.
