Where it usually breaks
And how to keep it from happening.
- A session is closed without the provider's encounter record.
- Evidence from the wrong date is verified.
- A request is attested while its evidence is still pending.
Schedule the consultation, close it on the provider's attestation and attest the service on verified clinical evidence.
Care Coordinator schedules the authorized request and records the session with provider, start date and duration. The request moves to Scheduled.
After the consult, the provider-attested session is submitted.
The Clinical Reviewer checks the revision, provider, timing, duration and encounter reference against verified evidence and approves the close, or rejects it.
Evidence lines arrive from the provider's clinical system and are marked Verified or Rejected.
On verified evidence, identity and revision are re-checked and the request moves to Attested with its date. Rejected or missing evidence leaves it unresolved.
