Assign and complete follow-up care

A completed encounter can still leave follow-up work for the care team. The follow-up workflow validates a task command, resolves the linked patient and assigned clinician, then routes the task through its supported lifecycle. Follow-up Coordinator reviews unresolved work while Veterinarian performs the assigned action under the appropriate access.

Step by step

  1. Define the follow-up Follow-up Coordinator

Follow-up Coordinator enters a task description and due date with the linked patient. The workflow rejects missing description or due information. Use a description that tells the receiving clinician what is expected rather than relying on a separate private conversation to explain the request.

    • Validate task description and due information
    • Updates Care tasks, Patients
  2. Resolve and assign the clinician Follow-up Coordinator

Follow-up Coordinator resolves the assigned clinician to the staff record and linked user account. The workflow confirms the patient and assigned clinician before routing. Re-confirm identity before recording the task as Open, as required by the workflow. A visible staff name alone does not establish who can receive the command.

    • Confirm the assignee before Open
    • Updates Care tasks, People, Users
  3. Work or escalate the task Veterinarian

Veterinarian starts the allowed in progress route and performs the follow-up. Use Escalated when the task needs further attention under the configured route. Cancelled is a distinct outcome. Agree how the team reads the task context and hands unresolved work to the receiving coordinator.

    • Route in progress, Escalated or Cancelled distinctly
    • Updates Care tasks
  4. Confirm completion Veterinarian

Veterinarian completes the permitted route with the required completion information, including completion date. Follow-up Coordinator checks status Completed and the recorded completion date. Missing completion information blocks that outcome rather than silently clearing the task from the workload.

    • Record Completed only with completion information
    • Updates Care tasks

Where it usually breaks

And how to keep it from happening.

  • A task lacks a due date or a useful description. Fix the input rather than accepting a partial assignment.
  • Assigned clinician identifies a person with no resolving user identity. Correct the mapping before routing work.
  • Completed is selected without a completion date. Inspect the workflow rejection and supply actual completion information.
  • Escalated or Cancelled is included with completed outcomes in a report. Keep the lifecycle states distinct when reviewing the workload.
