Step by step

1. Receive report Intake Coordinator

   The Intake Coordinator records the concern, organization and confidentiality scope, then checks its link to the matter. A report code can help authorized staff identify the case without using a person's name in the working queue.

2. Submit for review Intake Coordinator

   The Intake Coordinator moves the report from draft to submitted and retains the source revision. The handoff names the matter and the question the reviewer needs to decide.

3. Inspect the case Independent Reviewer

   The Independent Reviewer reads the submitted report and the permitted matter evidence. If a fact is missing, the reviewer records why the case needs more work.

   • Gives the reviewer case context

4. Record decision Independent Reviewer

   The Independent Reviewer approves, rejects or returns the source revision with a date and reason. A decision applies to the revision actually considered.

5. Reconcile effect Ethics Manager

   The Ethics Manager checks the guarded action receipt and its effect references. A failed or needs-review outcome remains open for reconciliation.

6. Complete report Ethics Manager

   The Ethics Manager confirms that the report outcome and matter record agree before treating the report as completed.
