What you can do

Make each observation reviewable

An accreditation cycle can contain evidence from different departments and reporting periods. Accreditation Management Operator records each observation against the cycle with a description and its source record identifier. Where a quantity is relevant, keep the unit beside it. The observation date explains when the evidence applies, while the source reference lets the team return to the underlying record.

Supporting documents remain connected to the same cycle. Their title, document date and external reference help the reviewer recognize the material. The attachment holds the supporting file, and the verification flag records whether its check is complete. The named verifier gives the preparation team someone to contact when a document needs clarification.

How the work is organized

Review the observation and the document as related parts of the evidence handoff. An accepted observation does not automatically explain every attached document. A verified file does not independently authorize the cycle’s accreditation decision. The reviewer examines the assembled evidence for the specific revision under review.

  • Keep source identity. Retain the original record identifier so the team can distinguish separate observations with similar descriptions.
  • Record the outcome. Use Pending, Accepted or Rejected to describe the evidence outcome and keep the reason available when further work is needed.
  • Check the document. Confirm the title and date against the attachment before recording verification under the responsible person.

What to agree during setup

Agree how the institution describes criterion evidence and which source references the reviewer will recognize. Decide what makes an observation suitable for the chosen standard, who checks its supporting document and how the preparation team records an unresolved issue. Keep quantity and unit conventions consistent across departments.

Prepare document examples with different dates and external references. Confirm that a reviewer can open the intended material and understand its relationship to the cycle. Establish how a replacement document is handled after a revision has been reviewed so the historical decision remains explainable.

Check the handoff on an actual case

A department submits an observation about completed activities and attaches a supporting summary. The observation date falls within the assessment period, but the attachment describes the preceding period. Accreditation Management Operator keeps the observation pending while asking the department for the correct material. The document remains awaiting verification until its content and date match the intended evidence.

When the correct summary arrives, the operator checks its external reference and records the verifier. Accreditation Management Reviewer then examines the cycle revision with the corrected evidence. If a further correction arrives after execution, use the documented revision correction process; keep the earlier document and decision available for comparison.

Rehearse this case with the person who prepares departmental evidence and the person who reviews it. Both should identify the same source observation and explain why the replacement was necessary before the cycle proceeds.

What your team sees

  • Pending criterion evidence — Find observations that still need a documented outcome.
  • Documents awaiting verification — Check supporting files against their dates, cycle and named verifier.

Follow the work through

Prepare and validate brings the evidence into review. Correct a delivered revision explains the later correction handoff.
