What you can do
Give continuing care an accountable owner
A completed encounter can leave care tasks that still need attention. Clinical Coordinator or Licensed Clinician records the task, patient and responsible person with a due date. The task retains its own state so the team can distinguish work that is Open, in progress, Completed, Escalated or Cancelled after the visit itself has ended.
Professional eligibility and care-team assignment are maintained separately. Clinical Governance Administrator checks clinician privilege evidence and records effective dates. A care-team assignment identifies the patient, clinician and access purpose for a defined period. These records support the clinic’s decisions about who should work with a patient and when.
How the work is organized
- Verify professional eligibility. Clinical Governance Administrator reviews the license number, issuing authority and supporting attachment for the privilege being checked. Pending becomes Verified after that decision. Keep Suspended and Expired distinguishable when eligibility changes, and review the effective dates alongside the state.
- Maintain the care team. Record the patient, clinician and access purpose with the assignment’s effective period. End an assignment when that responsibility finishes. Keeping the earlier assignment recognizable helps the team explain a change in care responsibility during a handover.
- Assign the next action. Clinical Coordinator or Licensed Clinician gives each care task a clear description and responsible person. Record the due date and update its state as work progresses. A completed task includes its completion date; unresolved work can be Escalated for further attention.
What to agree during setup
Agree the evidence needed for each privilege type and who performs the verification. Confirm how staff accounts relate to the people in the clinic, including the handling of inactive staff. Review care-team access purposes with the clinical owner and define how a changed assignment affects the person’s permitted work.
Choose how the team reviews overdue tasks and who handles an escalation. A task’s due date is a commitment to act, while its completion date records what happened. Decide how a cancelled task is explained during the clinical handoff, especially when the patient still needs a different next action.
Check the handoff on an actual case
Use a patient whose assigned clinician will be away. Clinical Governance Administrator reviews the receiving clinician’s eligibility and establishes the appropriate care-team assignment. Clinical Coordinator checks each open care task and records the person responsible for the next action. The receiving clinician opens their own task list and confirms the due dates.
Repeat the exercise with a privilege marked Suspended. Ask the governance administrator and clinical owner to resolve who can take the work under the clinic’s access policy. They should be able to explain the decision using the privilege evidence and assignment dates before a clinician acts.
Finish a follow-up task and record its completion date. Leave another unresolved task Escalated. During the handover, the team should be able to explain why the encounter is completed while that task still needs attention, and name the person responsible for the next step.
