What you can do

Make the assessment handoff visible

An encounter can require an assessment that progresses beyond the initial consultation. Licensed Clinician records the assessment type and accession number against the visit, then updates its state as the work happens. Collection has a dated record, while the processing state distinguishes Ordered, Collected, Received, Tested and Rejected work.

This gives the next person a practical question to answer: where is the assessment now, and what needs to happen before review? Clinical Reviewer uses the encounter context and associated clinical notes when considering the evidence. The assessment’s processing label describes the work performed; practitioners interpret the clinical information in the care record.

How the work is organized

  • Identify the assessment. Licensed Clinician chooses a clear assessment type and records the accession number used for the handoff. Link it to the encounter that requested the work. Recheck the patient through that encounter before collecting or recording anything against the assessment.
  • Follow the movement. Move Ordered work to Collected when collection takes place and record the collection date. Use Received when the item reaches the next processing step. A later Tested state records that processing has occurred, keeping the operational sequence understandable to the reviewer.
  • Expose exceptions. Use Rejected when an assessment cannot be accepted for processing under the practice’s procedure. Keep that record available for review and explain the clinical consequence in the encounter documentation. The responsible clinician decides what further assessment work the patient needs.

What to agree during setup

Agree the meaning of each assessment type and the source of its accession number. Define who records collection and who confirms receipt. Use the same naming convention when a result or supporting document arrives from a connected assessment service, so the clinician can find the corresponding encounter without guessing from a name alone.

Walk through the distinction between Received and Tested with the team. Choose the evidence staff need before updating either state and the route for handling a rejection. Results from an external service come from that service through a connection to set up; the practitioner reviews their meaning before incorporating them into the clinical account.

Check the handoff on an actual case

Create a practice assessment for a consultation and record its accession number while it is Ordered. At collection, the Licensed Clinician checks the encounter, records the collection date and marks it Collected. The receiving step then confirms Received. Ask Clinical Reviewer to locate the same assessment from the encounter and explain its current position.

Repeat the case with an item that is rejected on receipt. Keep the rejected record identifiable and review the associated note before deciding the next action. Do not silently use the same accession number for a different patient’s assessment. If further work is needed, the clinician identifies it explicitly during the handoff.

Finish by viewing Tested and Rejected assessments together. The team should be able to explain why each reached its state and locate the relevant encounter. That review helps catch an accession entered against the wrong visit before the clinical review proceeds.

What your team sees

  • Assessment processing queue — Find assessments still moving through ordering, collection and receipt.
  • Assessment workboard — Compare processing states and locate rejected work for investigation.
