What you can do
Check the purpose and the person
A patient consent records the purpose, version, signed date, expiry, document, and status. It belongs to one patient. The patient record also has a consent status, which helps the team locate people who need attention. For a real care decision, read the purpose-specific consent record and its document. A patient marked Active does not identify which purpose or version was signed.
Clinical privileges belong to a clinician. Each record carries a privilege type, license number, issuing authority, verification evidence, effective dates, and a status. Clinical Governance Administrator maintains these records and the care-team assignments that say which clinician is active for a patient and why. Together, those records help the team review the patient, person, and date involved in care.
How the work is organized
- Consent. Clinical Coordinator and Licensed Clinician maintain a patient’s purpose-specific record and signed document.
- Privileges. Clinical Governance Administrator records verification evidence and changes verification status.
- Care-team scope. Clinical Governance Administrator records patient assignment, purpose, dates, and Active or Ended status.
What to agree during setup
Agree on the wording and versioning of each consent purpose. Decide who checks that the signed document belongs to the right patient and version before changing the record to Signed. Decide how to handle expiry and withdrawal in day-to-day work. A later consent version should be clear enough that staff do not confuse an earlier signed document with the current purpose.
For clinicians, agree who inspects the issuing authority and evidence, who enters the effective dates, and when Pending becomes Verified. Suspended and Expired are distinct recorded outcomes. The person making the verification decision should be identifiable in the clinic’s operating procedure. Also agree how an ended care-team assignment affects access to a patient case.
Check the handoff on an actual case
Consider a patient who signs a consent for a specific purpose. Clinical Coordinator records the purpose and version, attaches the document, and enters the signed date. Licensed Clinician can find the consent when preparing the encounter. If the patient later withdraws, the consent moves to Withdrawn and the patient status can be reviewed separately. Staff should not reuse the earlier signed state for a different purpose.
Now consider a clinician whose privilege is Pending while evidence is checked. Clinical Governance Administrator reviews license details, issuing authority, evidence, and effective dates. After verification, the status is Verified. When the dated authority ends, the record can be marked Expired. Rehearse a visit near that date and have the team check both the clinician’s privilege and the patient’s care-team assignment.
