What you can do
Record exactly what was dispensed
A pharmacy dispensation belongs to an encounter. It records the medication, lot number, quantity, unit, and authorizing person. Licensed Clinician can capture this evidence during care, and Clinical Reviewer can read it with the visit. Because the record points to the encounter, the team can review the patient and clinician context when checking a dispensation later.
The lot number and unit matter as much as the medication name. A quantity without a unit can be misunderstood, and a medication without a lot loses the batch detail needed for a practical trace. The authorizing person is a People record, allowing the team to confirm who was named on the dispensation. Clinical privileges include a Prescribe type, with verification state and effective dates that can be checked during the operating handoff.
How the work is organized
- Visit context. Licensed Clinician selects the encounter before recording medication evidence.
- Dispensation. The entry captures medication, lot, quantity, unit, and authorizer.
- Review. Clinical Reviewer can inspect the entry with its visit and clinician context.
What to agree during setup
Agree the medication naming convention and how staff enter units. Decide who checks lot numbers against the item being handled and who confirms the authorizing person. A repeated entry for the same encounter should be investigated before staff add another record. The dispensation gives the team medication and lot evidence. Connect the pharmacy system the clinic uses for stock and fulfillment.
Clinical Governance Administrator should maintain the clinician’s verification evidence and effective dates. When Prescribe is the relevant privilege type, the team can check that record during its documented approval practice. Set the handoff with pharmacy staff so a clinician’s authorization and the actual medication issued remain connected to the same encounter.
Check the handoff on an actual case
During an encounter, Licensed Clinician records a medication with its lot, quantity, unit, and authorizer. Another team member checks the encounter’s patient and visit date, then reads the dispensation entry back against the physical item. Clinical Reviewer can later inspect that same evidence with the note and observations. The team should be able to find the exact batch without guessing from the medication name.
For a second case, the proposed authorizer has a privilege record that is Pending or Expired. Clinical Governance Administrator checks verification evidence and effective dates before the care team relies on the person for that privilege. Correct the person or complete the verification in the clinic’s operating process before finalizing the recorded dispensation. Rehearse the distinction between the person who enters the record and the person named as authorizer.
