What you can do

Start with the household

The household record is the anchor for visits, consent, payments and follow-up. The Clinical Coordinator enters the patient number, name, birth date and contact phone, then chooses an assigned worker. That assignment makes a named person responsible for the ongoing relationship. The team can read the current consent state before arranging a visit and can find earlier care from the same household record.

A community program may have several workers visiting the same area. Keep the assigned worker on each household current rather than relying on informal territory lists. When the responsible person changes, review upcoming visits and open follow-up actions separately; those records each carry their own worker reference. This preserves who was assigned to each piece of work.

How the work is organized

  • Patient identity. The patient number and name identify the household across visits and consent records.
  • Care access. A care team assignment records the worker, purpose and effective period.
  • Worker directory. People links a named worker to a user account and records whether that person is active.

The Clinical Governance Administrator maintains care team assignments. An active assignment can carry an access purpose and effective dates. Ending an assignment changes its status while retaining the record of the earlier access period. The coordinator keeps the household’s current worker aligned with the work that remains open.

What to agree during setup

Decide how patient numbers are issued and which coordinator checks duplicate identities before intake. Agree who can change an assigned worker and how the team handles an ended care assignment while a visit is still scheduled. Define the access purpose language your governance team uses. These are operational choices that shape reliable records, especially when a worker transfers or a household moves between teams.

For a pilot, load a small set of households and staff first. Confirm that each active worker has the right user account. Then link care assignments and check the household record as the coordinator and clinician.

Check the handoff on an actual case

A coordinator receives a request for a new household visit. They locate the household by patient number, check its contact phone and assign a worker. The governance administrator records an active care team assignment for that worker with an effective date and access purpose. The coordinator schedules the visit with the same household and worker. If coverage later changes, the team ends the earlier assignment and records the new one, then reviews the future visit and open action owners. The household history remains connected even though responsibility changes.

What your team sees

  • Household registry — Find a household by patient number and inspect its assigned worker and consent state.
  • My households — Give a worker a focused list of households assigned to them.
  • Scheduled visits — Check whether the next visit follows the current assignment.
