What you can do
Give follow-up work a patient and an owner
A clinician or coordinator assigns a task to a named person against the patient, with a due date and status. The assignee works from My Care Tasks. Open Care Tasks lets the coordinator see work still open, in progress or escalated. Care-team assignments record the clinician, patient, stated access purpose and effective period for the team to review.
How the work is organized
- Patient task. Give each action a patient, assignee, due date and current status.
- Access period. Record the clinician, purpose and dates on a care-team assignment.
- Escalation. Keep unresolved tasks in the coordinator’s open-work list.
A care task is attached to a patient, while a care-team assignment records why a clinician is associated with that patient and for how long. The two records answer different questions. My Care Tasks identifies the next action for an assignee; Open Care Tasks lets the coordinator find work that is still open, in progress or escalated. Completing a task should include the completion date so the team can distinguish finished work from an abandoned item.
What to agree during setup
Agree task wording, due-date practice and the meaning of escalation before the first live cycle. Name the person responsible for keeping care-team access current when assignments end. A task should describe a concrete next action that the assignee can complete and record.
Use a patient who needs a call after the visit. Give the task to a named person with a due date, then check both working lists. Add an escalated example and agree who will decide the next action. Review an assignment whose effective period has ended so staff do not confuse historical access context with a current care responsibility.
Check the handoff on an actual case
After a consultation, create a follow-up task for the patient with a due date and assigned person. Confirm the task appears in My Care Tasks for that assignee. Move it into progress and then complete it with a completion date. Check the coordinator view after each change; an escalated task should remain visible for attention.
Ask the assignee to open My Care Tasks and explain the action due for that patient. Have the Clinical Coordinator check the same item in Open Care Tasks and confirm that escalation remains visible. After completion, verify the completion date and that the task no longer appears as open work. Check the assignment record separately when responsibility for the patient changes.
