What you can do
Capture what was dispensed during care
A medication dispensation record links the encounter to the medicine, lot number, quantity, unit and authorizing person. It lets a reviewer see what was recorded for that visit without searching a free-text note. The lot and quantity provide a concrete reconciliation point when the clinic checks a physical handoff or investigates a later question.
The Licensed Clinician records the event under the visit. The authorizing person is a separate reference, so the clinic can distinguish the person who entered the record from the person named as responsible for the dispensation. The clinic should define its own prescribing and dispensing policy before using the records in live care.
How the work is organized
- Visit link. The record belongs to a specific encounter and can be read with the other care evidence from that visit.
- Medicine identity. The medication text, lot number and unit give the team a consistent description of what changed hands.
- Authorization reference. The named person provides an accountable point for review and correction.
What to agree during setup
Agree medication naming conventions and units so identical products are not recorded in several ways. Decide who checks the physical lot number and quantity before saving the event. Define when a clinician may be named as the authorizing person and how staff verify that person’s active privilege. If inventory or pharmacy software supplies stock balances, plan that connection separately.
For corrections, agree whether the team records a new explanatory note or changes the dispensation record under controlled access. A lot number or quantity error can affect a later patient inquiry, so the correction path should be rehearsed on a sample visit. Decide how the reviewer will compare the entry with the signed clinical note.
Check the handoff on an actual case
After an eye care visit, a clinician dispenses a medication used in the clinic’s care plan. The clinician enters the medication name, the lot read from the package, the quantity, the unit and the authorizing person. The record points to the encounter that contains the clinical note and observations. During review, the Clinical Reviewer can inspect the linked dispensation and ask the clinician to correct an inconsistent lot or quantity before closing the visit.
The coordinator should not infer payment or charge status from a dispensation alone. If a billable service is involved, the charge is recorded separately against the encounter and follows the clinic’s billing review.
