What you can do
Let a reviewer make a bounded decision
An independent reviewer needs to know which assessment revision they are judging and why they reached a decision. The decision record names the reviewer, source revision, decision, reason and date. It belongs to an assessment, but approval of a review record does not by itself change the assessment outcome. The Assessment Owner still needs to reconcile tests, findings and corrective work before closing an engagement.
Retesting is another separate judgement. A retest belongs to a finding and records the reviewer, date, evidence and pass, fail or inconclusive result. The action owner cannot act as the independent reviewer on their own correction. Only a passing retest verifies the action and closes the finding. A failed or inconclusive retest keeps the finding open for more work.
How the work is organized
- Decision history. Source revision, reviewer, reason and date explain the assurance response.
- Retest record. A distinct result and evidence show whether corrective work addressed the finding.
- Operation record. Receipts preserve an operation key, expected revision, effect and failure reason when processing needs review.
What to agree during setup
Choose independent reviewers who are outside the ownership of the action they inspect. Define what a substantive decision reason looks like for approval, rejection and return. Make sure the reviewer can locate the assessment revision and supporting attachments before recording a decision. If an assessment changes after review, the next decision should identify the new source revision rather than silently replacing the old judgement.
Decide who inspects failed or review-needed operation receipts. A repeated request with the same operation key should be recognizable, and a revision mismatch deserves investigation before anyone retries the action. The receipt is operational evidence; it does not substitute for a person’s assurance decision.
Check the handoff on an actual case
Give an Independent Reviewer a submitted campus review, the parent assessment and a finding with submitted action evidence. Have them record a reasoned review decision against the assessment revision, then perform a retest as a separate step. Record the campus context in the reason so a later reader can distinguish it from another review under the same assessment. Confirm the review decision leaves the parent assessment’s outcome to its owner, while the retest result governs the finding. Finally inspect any operation receipt for the attempted change and resolve failures before closing the cycle.
What your team sees
- Retests Requiring Attention — Failed and inconclusive retests that keep findings open.
- Operations Needing Review — Failed or review-needed operations with their reason and assessment.
- Assessments in Review — Engagements awaiting independent judgement.
Follow the work through
Follow campus assurance review for the decision and remediation and retest for independent verification.
What it keeps track of
Data model- Review decisions
- Independent reviewer decisions recorded against an assessment revision.
- Retests
- Independent retests that decide whether a finding may be closed.
- Operation receipts
- Idempotent execution receipts for guarded operations on assessments.
- Assessments
- Scoped assessment engagements driven through a status lifecycle.
- Findings
- Findings raised from failed or inconclusive control testing.
- Corrective actions
- Remediation actions assigned to a finding owner with due dates and evidence.
- Record attachments
- Evidence files attached to an assessment with a visibility scope.
