Pilot one outcome measure for one organization across a few monthly windows. Build, review and publish each cohort, and reconcile the published rates with the figures your team already reports.

• The measure specification, with inclusion rules, numerator, denominator, grain and period.
• A deidentified extract with stable source references and a consent basis for each fact.
• The people who will act as Metric Steward, Outcomes Analyst and Clinical Reviewer, with their email addresses.
• The rates you previously published for the pilot measure.
• Your suppression threshold and the evidence a reviewer expects.

A cohort Owner must not approve their own revision. A definition’s author must not approve their own draft. A repeated start with the same operation key must return the existing run. A publication run must not succeed before its read-back. A zero denominator must produce a suppressed result with a reason. Activate and rehearse the correction workflow before you rely on it. Invite the pilot team using the agreed roles, confirm the approval owners and release the reviewed configuration. Expand only after the first cycle reconciles with your source.
