What you can do

Keep the work in its source record

Begin with an organization, a subject, an owner and a period. The coordinator sets the assessment to Scheduled, then records Arrived and in progress as work begins. The period dates make the planned window visible beside the actual start.

How the work is organized

  • Set the period. The coordinator chooses the organization, subject, planned dates and assigned team member on the assessment.
  • Attach results. The clinician gives every measured result the correct assessment parent and a team member owner.
  • Move the assessment. The coordinator checks the current work before changing its operational state.

The clinician records each measured result against its parent assessment, with the metric, value, unit and test date. The owner on the result makes follow-up attributable. Keep a result tied to the period that produced it so the reviewer can inspect the right evidence.

What to agree during setup

Agree who changes the assessment state, when work is ready for review, and how a cancelled period is recorded. Choose a real assessment and compare its period, actual start and result dates before use. Walk through a completed case and a correction with the people who will operate the record. Decide where source documents arrive and who checks them before a status is changed. Keep the agreement close to the team’s operating procedure so a new owner can make the same decision.

Check the handoff on an actual case

A coordinator schedules a care team review for an organization and assigns a team member. The clinician enters a dated measurement. When the work is ready, the coordinator moves the assessment to awaiting review. The reviewer opens that assessment and checks that its results belong to it before a decision is signed. Reopen the assessment with the reviewer and compare each result parent with the assessment ID. Check the measurement date and unit, then read the reason on the related decision. If a result belongs to another period, correct its parent before the coordinator completes this assessment.

When the period ends, compare the final state with the dated results and the decision. A Completed assessment should be explainable from its linked records. A Cancelled one should remain findable, because a later owner may need to understand why planned work stopped. Keep the subject specific enough to distinguish recurring periods for the same organization.

What your team sees

  • Assessment schedule — Open the working records that need the module’s first decision.
  • Assessments awaiting review — Compare related work before handing the case to another owner.

Follow the work through

Follow assessment to decision and follow-up and retest. Set ownership in roles before rehearsing the handoff.
