What you can do

Keep specialty evidence on the encounter

A specimen record names its encounter and specimen type. Its accession number, collection date and status help the team follow the item from ordered through collected, received and tested, or record a rejected outcome. A pharmacy dispensation names the medication, lot number, quantity, unit, encounter and authorizing person. These references put specialty evidence beside the visit it belongs to.

The Licensed Clinician can manage these records. The Clinical Reviewer can read them during encounter review. A specimen may still be in progress after the visit’s other documentation is ready, so the team should decide when its result matters for closing the encounter. The recorded specimen status gives the coordinator and clinician a shared point of reference for that handoff.

How the work is organized

  • Specimen progress. Record the specimen type, collection time and accession number against the encounter as processing advances.
  • Dispensation detail. Record medication, lot, quantity and unit with the authorizing person.
  • Review context. The reviewer reads specialty records with the encounter’s notes and observations.

What to agree during setup

Choose a convention for accession numbers that lets a team compare a specimen with its laboratory source. Agree when a collected item becomes received and who records a rejected result. For dispensations, decide how lot and unit values are entered and how the authorizing person is selected. Check that the person record corresponds to the clinician responsible for that action.

A laboratory or pharmacy system can supply additional results or fulfillment information through a connection. Plan that mapping around the encounter and accession or lot reference so the same case can be traced across systems. Keep the meaning of each status clear to the care team before a busy operating day.

Check the handoff on an actual case

Open a patient’s in-progress encounter. Record a specimen type and collection date, then enter its accession number when available. Record a dispensation with medication, lot, quantity, unit and authorizing person. The reviewer opens the encounter and checks that both items belong to that visit, and that the specimen’s current state matches the team’s handoff.

When a specimen is marked tested, keep its accession number visible in the encounter review so a reader can connect the processing state to the original collection. When an item is rejected, check the encounter and specimen type before planning another collection. The visit record provides the context for that decision.

What your team sees

  • Specimens in Process — Ordered, collected and received specimens with encounter and accession details.
  • Visits Awaiting Review — Encounters whose specialty evidence can be read with notes and observations.
