What you can do

Put the authority beside the work

A care team assignment names a clinician, patient, access purpose, effective dates and status. Clinical Lead uses it to show why that clinician is involved in the patient’s care during a particular period. An ended assignment should not be treated as current responsibility. The assignment is a patient-level record; an encounter still names the clinician working that particular visit. Staff should review both when handing care to another person.

Clinical privileges record the clinician, privilege type, license number, issuing authority, evidence, effective dates and status. Admin keeps verification evidence current; a Verified privilege is different from Pending, Suspended or Expired. Before documentation, prescribing or review, the relevant privilege type should be checked against the intended action. Consent records similarly carry their purpose, version, signed document and dates. A signed consent for one purpose does not answer a different decision by its label alone.

How the work is organized

  • Care access. Clinical Lead maintains patient-specific clinician assignments and effective periods.
  • Professional authority. Admin records privilege evidence and verification state for each clinician.
  • Patient decision. Admin and the treating team maintain the consent document, purpose and status.

What to agree during setup

Agree who verifies professional evidence, how often it is reviewed, and how staff learn when it expires. Decide which actions require Document, Prescribe, Review or study investigator privilege. Set a procedure for suspending a privilege and reassigning affected care. Define consent purposes and versions in terms the patient and clinical team can recognize. Determine who records a withdrawal and who checks its impact on scheduled care. Configure staff accounts and the directory relationship so the person in the care record is tied to the appropriate user identity.

Check the handoff on an actual case

A patient is assigned a clinician for an active period. Clinical Lead records the access purpose and effective dates. Admin checks that clinician’s professional evidence and marks the relevant privilege Verified. Before the clinician signs a medication order, the team checks the prescribing privilege and the patient’s applicable consent. If a colleague takes over, Clinical Lead ends the earlier care team assignment and enters the new one. The visit remains connected to the correct patient and clinician record.

If the patient’s consent is Withdrawn or the clinician’s privilege is Suspended, stop the affected decision and bring it to the clinical lead. The statuses make the issue visible but staff still need to determine the appropriate next action. At review time, look at purpose and dates, not only a current-looking label. Retain the earlier assignment and consent record as context for the care already delivered.

What your team sees

  • My encounters — Let clinicians find the visits assigned to them before acting.
  • Encounter review queue — Bring pending clinical decisions into view for the lead.
