What you can do
Connect permission to a purpose
Clinical Coordinator records a purpose-specific consent for the patient. The record carries the document, version, signing date, expiry date and current state. This gives the care team a place to examine the actual consent evidence when arranging an encounter. The patient’s own consent status offers a broader identity-level signal; the purpose-specific document explains what was signed and when.
Clinical Governance Administrator maintains care team assignments. Each assignment connects a patient to a clinician, gives the access purpose and identifies the effective window. Active and ended assignments can therefore be reviewed separately. The administrator also maintains clinician privileges, including privilege type, license details, issuing authority, verification evidence and effective dates. A pending privilege calls for human review before it is marked verified.
How the work is organized
- Consent record. Purpose and version distinguish one signed document from another; expiry and withdrawal keep the current state visible.
- Care assignment. The patient, clinician and access purpose identify why a person belongs to that patient’s care team.
- Privilege review. The administrator checks license evidence and dates for the relevant privilege type before recording the verification decision.
What to agree during setup
Define the consent purposes your team will record and the version naming used when a document changes. Decide who obtains the signed document, who enters its dates and who follows up on pending or expired consent. A signature date and attached document need to refer to the same version. If a patient withdraws consent, the team should review affected work in its normal clinical process.
Set the expected evidence for each privilege type and identify who is allowed to verify it. Choose how the team reviews privileges approaching their effective end date. Care team assignments need an access purpose and effective period that staff can understand later; avoid using an active assignment as a substitute for verified professional privilege.
Check the handoff on an actual case
Before a scheduled encounter, the coordinator checks the patient’s consent record and sees that the relevant purpose is signed. The governance administrator checks the clinician’s license and evidence, records the verified privilege and maintains an active assignment for that patient. The clinician can then look at the episode context, while the reviewer can see the consent and privilege evidence tied to the people involved. An expired consent or suspended privilege remains visible for the team to resolve before proceeding.
