What you can do

Follow a sample through processing

The Licensed Clinician can record a tissue sample for a donation event. The sample record carries its type, collection date, accession number and current status. Ordered, Collected, Received, Tested and Rejected give the team a compact processing trail. The event reference keeps the sample with the donor episode that produced it. A sample marked Received is still different from one marked Tested; the current status should reflect the last observed processing step.

Medication dispensations are also linked to events. Record the medication, lot number, quantity, unit and authorizing team member. These details let the clinical team identify what was dispensed in the context of the event. A dispensation record is evidence of the recorded action; staff should confirm the entry against their clinical procedure before treating it as complete.

How the work is organized

  • Sample identity. Keep the accession number and sample type with the originating event.
  • Processing state. Update the sample status as collection and testing evidence arrives.
  • Dispensation detail. Record the medication, lot, quantity, unit and authorizing person together.

What to agree during setup

Decide how accession numbers are issued and how staff check for an existing sample before entering another. Agree who records collection and receipt, and how a rejected sample is documented. A sample can be rejected after collection or receipt, so the operational review should examine the event and accession number rather than counting only the final status. If a laboratory produces the test result elsewhere, set up the exchange and review responsibility explicitly.

For medication, agree on the source for lot details and the expected units. Train the team to enter the authorizing team member on the same event. Check a case with two dispensations to make sure they remain separate entries with their own quantities and lots. The Clinical Reviewer should be able to read them without inferring an approval or inventory deduction from the presence of a dispensation record.

Check the handoff on an actual case

During an event, the clinician records a sample as Ordered, then enters its collection date and accession number when collected. The team sets the sample status to Received when it has the corresponding evidence. The status becomes Tested only after the result is available to the responsible staff. In the same event, a medication dispensation records a measured quantity, unit, lot and authorizing team member.

Rehearse a second sample that is Rejected. Confirm it remains linked to the event and can be found in a processing review. Ask the reviewer to compare sample state, medication details and the event note, looking for discrepancies that the clinician should correct before the event moves to completion.

What your team sees

  • Samples in processing — Ordered, collected and received samples with accession details.
  • Events awaiting review — Event cases where sample and dispensation records can be inspected together.
