What you can do

Clinical evidence in daily work

Evidence from the clinical system is linked to its care request. Each line can carry an external record ID, provider, clinical system, evidence type, occurrence date, revision and verification status. Attached files can carry an evidence type and visibility.

Evidence starts Pending and is checked against the provider and request identity. Verified evidence supports attestation. Rejected or missing evidence remains unresolved. The attested date on the request follows a confirmed match rather than a simple upload.

How the work is organized

  • Source. The Operations Manager compares the provider, external record ID and revision with the parent request. A file upload alone does not establish verified service.
  • Decision. A verified line supports completion only for its matching request. Keep a rejected line visible while the provider supplies corrected evidence.
  • Follow-up. The reviewer checks the evidence used for attestation. The Operations Manager follows any failed receipt to its original operation key.

What to agree during setup

Agree which clinical system is authoritative and how its external IDs are preserved. Set rules for attachment visibility and who may upload material that is shared outside the practice.

Confirm which team member can act for the practice and how staff check the current revision. Rehearse a changed case with the person who receives this handoff.

Check the handoff on an actual case

Take a completed provider visit with one matching evidence line and one line for another patient or revision. Confirm only the matching line supports attestation; retain the rejected line and its reason for investigation.

Have the next person open the linked request and explain what changed, who decided, and which evidence supports the current state. Repeat with a stale revision; the team should return to the current record before acting.

Evidence may arrive as a line from the clinical system or as an attached file. Keep its external record identity and occurrence date with the request. When a line is rejected, the team should find the specific line and ask the provider for a correction rather than treating every attachment as proof of service.

For a completed visit, compare the evidence line with the request before changing its verification result. The external record ID helps the provider locate the source clinical entry; the request link tells the practice which case it belongs to. Keep the occurred date with the line when staff investigate a delayed arrival.

What your team sees

  • Verified Clinical Evidence — Evidence lines checked against the source request and provider.
  • Evidence Pending Verification — New lines whose identity and revision still need checking.
