What you can do
Provider coordination in daily work
The coordinator selects a provider organization associated with the practice. The provider record holds its name, code, active state, contact and eligibility note. The care request points to that provider as its counterparty.
Authorization comes before scheduling. The coordinator checks availability with the provider and then moves an authorized request to Scheduled within the confirmed arrangement. The provider remains responsible for its availability; the request records the coordination outcome.
How the work is organized
- Source. Admin maintains each provider organization under the practice, including its contact, active state and eligibility note.
- Decision. Care Coordinator confirms availability with the provider for the authorized scope before scheduling. The provider remains the authority for its own availability.
- Follow-up. A substitute provider can change the reviewed scope. Compare the current revision and ask the Authorized Clinical Reviewer to assess the change before proceeding.
What to agree during setup
Agree who can maintain provider eligibility notes and active state. Define how a confirmed availability response is captured and who handles a provider substitution after professional review.
Confirm which team member can act for the practice and how staff check the current revision. Rehearse a changed case with the person who receives this handoff.
Check the handoff on an actual case
Use a case where the initial provider cannot take the requested units. Keep the original reviewed scope visible, check a replacement provider relationship and make the reviewer examine any change that affects the authorized scope.
Have the next person open the linked request and explain what changed, who decided, and which evidence supports the current state. Repeat with a stale revision; the team should return to the current record before acting.
The provider contact and eligibility note help the coordinator reach the right organization when an authorized request is ready. If the active state changes while a request is in flight, check the relationship with Admin and the reviewer before moving the request forward. Keep the existing reviewed record available.
The provider organization record has a contact name and eligibility note, but availability is confirmed by the provider for a particular request. A provider being active does not itself reserve time. Keep the request on its authorized scope while the coordinator checks the response and sends any material change back for review.
