What you can do

Review the exact report that will be released

Report detail belongs to the parent imaging order and carries its patient, accession, DICOM study and PACS references. The operator prepares each line with its sequence, description, observation date and outcome. A reviewer sees the order revision, the report lines and supporting evidence before recording a decision. The reviewer is independent of the preparer; approval is a decision on that revision, not a standing permission for later changes.

An accepted report outcome and an approved order revision answer different questions. The report line records the finding’s review outcome; the decision record captures reviewer, decision date, reason and evidence for the order revision. The authorized radiologist is the release authority. The release step should preserve confirmed external receipt evidence with the operation step that produced it.

How the work is organized

  • Prepare matching lines. Link each report line to the parent order, patient, accession and DICOM study.
  • Decide on the revision. The independent reviewer records Approved, Returned or Rejected for the exact order revision.
  • Confirm release. The authorized radiologist signs the reviewed study and the operation step retains its receipt.

What to agree during setup

Agree who prepares report lines and who independently reviews the order revision. Decide what evidence the reviewer needs when returning a case and how the operator submits a corrected successor revision. Confirm the radiologist assigned to the order and what external acknowledgement is enough to establish release. Rehearse a mismatch between a line’s study identifier and its parent order.

Check the handoff on an actual case

Consider a study with two report lines. The operator checks that both lines reference the same parent order and matching accession and study identifiers. The reviewer returns the order revision because one line needs correction and records the reason against that revision. The operator updates the report and submits the successor revision. Only after independent approval does the authorized radiologist sign and release the report. The earlier decision and execution history remain available to explain the change.

The reviewer returns the order revision because one line needs correction, recording a reason and evidence reference in the decision. The operator fixes the line and submits a successor revision for another independent decision. Before release, compare the patient’s identity, accession and DICOM study across order and report. Keep the earlier decision visible. The authorized radiologist signs only after approval of the current revision and a confirmed release receipt is retained.

What your team sees

  • Pending Review Queue — Find report lines still awaiting an outcome and inspect their parent study.
  • Imaging review decisions — Inspect pending and returned decisions by source revision.
