Click Get started, then use Proto to configure Safety / Incident Reporting for your business. Start with one operating unit, an assessment period and a real incident to rehearse the review.
Link the investigation to its parent assessment and organization. Record the site and occurrence in the investigation itself. Find the action through the finding under that assessment.
The Safety Manager sets the period, scope and owner. The owner follows testing, findings and remediation through the assessment.
Record the sample size, exception count, Fail result and evidence reference in the test. Open a finding linked to the same control and assessment.
The Action Owner submits completion evidence. An Independent Reviewer retests the finding, and the team checks the result before treating the action as Verified.
Keep the failed result attached to the finding and send the response back for further work. The original test and action remain part of the case.
The Independent Reviewer records Approve, Reject or Return with a reason and source revision under the assessment. Check the investigation revision before relying on the decision.
Attach supporting files to the assessment, label their evidence type and choose Internal or Shared visibility for the intended audience.
Use each assessment's organization, operating unit and period, then inspect the linked tests, findings and decisions before comparing outcomes.
Agree any document, identity or external reporting transfer during implementation. Keep ownership of external acknowledgements clear.
Start free, then use Proto to change fields, steps and approvals before your team goes live.
