How a Safety / Incident Reporting rollout works

Start with a bounded incident and action review cycle.

  1. Set the scope

Choose an organization, active operating unit and assessment period.

  2. Prepare ownership

Add team members, risk and control owners, an Investigator, an Action Owner and an Independent Reviewer.

  3. Record a case

Enter an incident or near miss under that assessment, then a control test, finding and correction with evidence.

  4. Rehearse review

Retest the finding, record a reasoned decision and compare it with the investigation status.

  5. Expand work

Add other operating units after the first team's handoff and evidence conventions are clear.

What the customer provides

Bring the operating-unit list, named team members, current hazard and control descriptions, sample assessment periods and an incident suitable for a pilot. Agree the scoring scale, evidence naming convention and who makes the final decision. Keep sensitive files under the visibility choice that matches their audience.

Changing it afterwards

When a control changes, record its current effective date and status while preserving older test references. When an incident account changes after submission, have the reviewer inspect the current revision. Review action ownership and due dates at each operating handoff. Use the data model when planning an import and roles when assigning access.
