What you can do

Make the next care task accountable

Licensed Clinician orders a care task for a patient and assigns it to the responsible care-team member. The task description identifies the work, and the due date gives the team a point for follow-up. Tasks can be open, in progress or escalated, so the team can distinguish routine work from a handoff needing attention.

The responsible clinician records completion evidence or escalates the task. Clinical Reviewer then closes eligible work through the authorized review action. A Completed task retains its completion date. This process coordinates the recorded follow-up; the clinical judgment behind the patient’s care remains with the authorized clinician.

How the work is organized

  • Patient and responsibility. The task names the patient and assigned person. Resolve that person to the intended care-team member before acting, especially when the task is opened from a personal work list rather than the patient record.
  • Progress and escalation. The responsible person keeps the current state understandable. An escalated task needs a clear handoff through the agreed review path so it does not disappear among ordinary open work.
  • Reviewed completion. Clinical Reviewer checks the eligible completion evidence under current professional and patient authority. The close is a guarded action; ordinary visibility of the queue does not grant unrestricted authority to finalize clinical work.

What to agree during setup

Agree what information belongs in the task description and which evidence supports completion. Clinical Coordinator can coordinate permitted operational work, but task content and related links must preserve the role’s clinical information boundaries. Keep clinical note text, observations and specialty results in their appropriately restricted records.

Define how the team handles a task approaching its due date and who receives an escalation. Agree the eligible review path and how the assigned clinician records that the work is ready for a decision. For cancellations or corrections, preserve the task’s context and use the authorized process rather than deleting the patient’s follow-up history.

Check the handoff on an actual case

Use a follow-up task created from a completed skincare encounter. Licensed Clinician records the task, selects the responsible care-team member and sets the due date. The assigned person begins the work and supplies the evidence required for review.

Have Clinical Reviewer inspect and close the eligible task. Confirm that the completion date is retained and that the reviewer had current access to this patient. Repeat with an unassigned reviewer and with a task that needs escalation instead of completion. The first request must be refused, and the escalated case should remain visible for the appropriate follow-through. Ask Clinical Coordinator to coordinate the permitted task context while confirming that the same identity cannot open linked restricted clinical evidence.

What your team sees

  • Care task queue — Open, active and escalated follow-up work with its patient and assigned person.
  • My Care Tasks — The Licensed Clinician’s assigned work and due dates.
  • Encounter board by status — Visit context for the care team’s authorized patient work.
