What you can do

Keep the clinical record attributable

Licensed Clinician records observations against the encounter with the measured value, unit and observation date. Each observation identifies the person who recorded it. Clinical notes retain the note type, text and author, giving the authorized reviewer a clear account of who documented the encounter.

A note starts in Draft. Signing belongs to the currently licensed authenticated clinician with the required authority, and the signed note records its signature date. Signed content remains immutable. Clinical Coordinator’s ability to admit or coordinate an encounter does not authorize the coordinator to write or sign clinical documentation on the clinician’s behalf.

How the work is organized

  • Encounter-linked observations. Each observation points to the encounter it describes. The metric, value and unit remain together so the reader can interpret the recorded measurement in its documented context without substituting an assumed unit.
  • Authored clinical notes. Licensed Clinician prepares the note under the correct author identity. The Draft state distinguishes work still being prepared from the version that has been signed under current professional authority.
  • Controlled review and amendment. Clinical Reviewer reads eligible patient evidence through the authorized review path. Signed content stays preserved, and any amendment follows the clinic’s explicitly authorized correction process with the signed source still available.

What to agree during setup

Agree the clinic’s note-type and observation-metric vocabulary and how units are entered. Define how the application resolves the acting platform user to the clinician record before accepting attribution. A selectable author name must not allow someone to impersonate another clinician for signing.

Set the review path for encounters that need an independent evidence decision. Review access remains limited to the appropriate patient assignment and professional authority. Agree the amendment process before the pilot so users know how to correct a signed record without editing its original content. Keep these clinical views separate from scheduling, governance administration and service billing access.

Check the handoff on an actual case

Use a permitted encounter for a patient already assigned to the treating clinician. Licensed Clinician enters an observation with its recorded unit and prepares a Draft note. Check that both records point to the correct encounter and identify the actual author or recorder.

Have the clinician review and sign the note. Confirm the signature date and attempt to alter the signed text through an ordinary record edit; the signed source must remain protected. Try signing as Clinical Coordinator and with a clinician whose required privilege is no longer valid. Both attempts must be refused. Finally, let Clinical Reviewer inspect the permitted evidence and attempt the same access for an unrelated patient. The review should remain limited to its authorized care-team scope.

What your team sees

  • Clinical notes list — Draft, signed and amended note records for authorized encounters.
  • Notes Awaiting Signature — Draft notes that need the appropriately authorized clinician’s attention.
  • Observation log — Attributed measurements with their metric, value, unit and observation date.
