What you can do
Keep specialty evidence with the patient encounter
Licensed Clinician records the skin assessment concern and assessment notes against the selected encounter. The assessment identifies the clinician and includes the photographic consent context. Submission checks the real linked records and professional authority before routing the evidence to the responsible reviewer.
Treatment photographs belong to an encounter and carry their capture date, image type and restricted access group. Classify each image as before, after or follow-up evidence. The image submission checks the linked patient and clinician, photographic consent, Document privilege and care-team coverage before the file is retained under restricted access.
How the work is organized
- Assessment attribution. The concern and assessment notes remain linked to the encounter and clinician who submitted them. Clinical Reviewer can inspect the permitted evidence without transferring access to operational or governance roles.
- Photographic purpose. The image category and capture date help the care team identify the evidence. The photographic consent check applies to the patient and action; a general patient consent summary is not enough to authorize the submission.
- Restricted image handling. The access group provides context for the image’s intended readers. The actual file retrieval path must enforce the allowed care-team and professional scope, including after consent or assignment changes affect a new access request.
What to agree during setup
Agree the assessment wording and the evidence needed for reviewer follow-through. Define the purpose and version used for photographic consent and how the team confirms that it applies to the selected patient and action. Keep the assessment’s consent text aligned with the authoritative consent process.
Agree how image types, capture dates and restricted access groups will be used. Rehearse image upload and retrieval with actual role identities. Clinical Coordinator can coordinate the encounter without seeing patient-level specialty evidence, and Clinical Governance Administrator can maintain authority records without opening the assessment or photographs. Any external image storage connection must preserve those access boundaries.
Check the handoff on an actual case
Use an encounter with valid photographic consent and an assigned Licensed Clinician holding the current Document privilege. Submit an assessment and a Before image, then verify the encounter and patient relationship from the stored evidence. Add the corresponding later image under its own capture date and appropriate category.
Repeat the upload with withdrawn photographic consent, an unrelated encounter and an expired care-team assignment. Each invalid request must be refused before evidence is saved under the wrong authority. Ask Clinical Reviewer to retrieve the permitted image through the actual file path, then attempt retrieval as Clinical Coordinator. The image should remain restricted even if its identifier or link is known. Keep the stored evidence descriptive; clinical interpretation remains with the authorized professionals reviewing the patient record.
