What you can do

Keep the clinical record attached to the right visit

A patient can return for several visits while earlier follow-up work remains open. The encounter gives each visit its own scheduled time, clinician and working state. Clinical Coordinator uses that context to distinguish an appointment that is still Scheduled from a patient who has Arrived or a visit already in progress.

Licensed Clinician adds clinical notes and observations to the encounter being worked. A note identifies its author and signing date. An observation identifies the person who recorded it, the measurement time and the unit. Clinical Reviewer can examine these records together before deciding whether the encounter is ready to complete.

How the work is organized

  • Coordinate the visit. Clinical Coordinator records the visit type and scheduled time, then updates the state as the patient arrives and care begins. Start and end times describe the actual encounter. Keep a cancellation separate from a completed visit so the working queue remains understandable.
  • Attribute the evidence. Licensed Clinician writes the note against the encounter and records measurements with their units. Draft notes remain distinguishable from Signed notes. An amendment uses Amended so the team can recognize that a signed account needed a later correction.
  • Request a decision. Clinical Coordinator moves the encounter to awaiting review when the evidence is ready. Clinical Reviewer examines the clinical account and identifies any unresolved questions. After the reviewer approves the outcome, the coordinator records Completed and the actual end time.

What to agree during setup

Choose visit type wording that reception and clinicians use consistently. Agree when an encounter begins, when its end time is recorded and what evidence the Clinical Reviewer expects before deciding. Establish the clinic’s note signing and correction procedure so authors know how to explain an amendment.

Decide which measurements the practice records and the unit used for each. When the unit changes, readers must be able to distinguish the observations. Define how review questions return to the Licensed Clinician and who checks that the coordinator has recorded the agreed outcome. Clinical Reviewer reads the evidence; Clinical Coordinator maintains the encounter state.

Check the handoff on an actual case

Rehearse a follow-up consultation for an established patient. Clinical Coordinator at reception marks the encounter Arrived. Clinical Coordinator records the start as the visit becomes in progress. Licensed Clinician enters the observations, checks the unit and signs the visit note under their own identity. The coordinator then requests review.

Have Clinical Reviewer find a measurement that needs explanation. Leave the encounter awaiting review while the clinician checks the source and adds the appropriate clarification. After review approval, the coordinator completes the encounter. Confirm that everyone can still distinguish the measurement time, note signing time and encounter end time.

Finally, repeat the handoff with a cancelled appointment. The coordinator should select Cancelled and keep it separate from completed care. Use that contrast when reviewing the day’s encounter states so an appointment outcome is never mistaken for evidence that care took place.

What your team sees

  • Encounter schedule — Compare scheduled visits with their start and end details.
  • My clinical notes — Return to notes attributed to the signed-in clinician.
  • Encounter review queue — Find visits waiting for the Clinical Reviewer’s decision.

Follow the work through

Follow intake to encounter review, then arrange the remaining work through care-team setup and follow-up.
