What you can do
Follow the evidence that leaves the consultation
Some encounters lead to specimen collection or a medication dispensation. The next person handling that work needs to identify the visit and understand what has already happened. Licensed Clinician links the specialty record to the encounter, records the relevant details and checks the connection before handing the work onward. This makes the patient’s visit the common reference for the clinical reviewer.
Specimen processing has a state that changes as work progresses. Medication dispensation records instead describe what was supplied, including the medication, lot, quantity, unit and authorizing clinician. Keep each record aligned with the event it describes. Laboratory results come from the center’s designated laboratory system; agree that connection and the responsible clinical review before the pilot.
How the work is organized
- Specimen identity. Record the specimen type and accession number against the encounter. Add the collection time when collection occurs. The accession number gives staff a useful reference for a laboratory inquiry, while the encounter identifies the visit that requested the work.
- Processing state. Licensed Clinician maintains Ordered, Collected, Received, Tested or Rejected as those events are confirmed. A rejected specimen needs attention even if another specimen from the same visit progresses. Keep each specimen’s actual result of processing visible during handoff.
- Dispensation detail. Record the medication and lot with the quantity and unit supplied. Identify the authorizing clinician and review the applicable prescribing privilege under the center’s policy. A quantity is meaningful with its unit, so compare matching units when reviewing dispensing activity.
What to agree during setup
Agree the specimen type labels and how the center receives confirmation of collection, laboratory receipt, testing or rejection. Identify the system that owns laboratory results and the person responsible for checking that its accession number matches the encounter record. Rehearse the response to an unreadable label or an accession number that cannot be matched confidently.
For dispensations, agree medication naming and the units used for recording quantities. Show staff where to find the lot number on the supplied item and how to identify the authorizing clinician. Decide who reviews corrections before they affect the handoff. Set up any external pharmacy or stock connection separately, with clear ownership of the inventory and purchasing records.
Check the handoff on an actual case
Use a fictional visit with a specimen recorded as Collected and an accession number ready for laboratory handoff. When receipt is confirmed, Licensed Clinician changes it to Received. Rehearse a rejected specimen separately so the team can identify which work needs attention. Then record a dispensation for the same encounter, with a sample medication label, lot and quantity expressed in a named unit. Clinical Reviewer should be able to follow both specialty records back to the intended visit and clinician.
