What you can do
Start with the correct donor
The Clinical Coordinator starts a collection by finding the donor and checking the recorded identity. The donor code, name, birth date and contact phone help the team distinguish people with similar names. The screening authority identifies the team member responsible for the donor’s screening record. A returning donor should lead the coordinator back to the same history before another collection is scheduled.
Consent has its own recorded state. Pending means the team still needs to resolve consent through its agreed procedure. Active records the current consent position, while Withdrawn makes a changed decision visible. The coordinator checks the supporting document alongside that state. A signed attachment and the donor’s current consent answer different questions, especially when consent changes after a document was signed.
How the work is organized
- Identity. The Clinical Coordinator maintains the donor’s identifying details and screening authority. Agree how the donor code is assigned and how staff handle a possible duplicate before importing a returning donor list.
- Consent. Record the donor’s current consent state after the responsible person completes the consent procedure. A collection discussion should use that current position even when older signed evidence remains available.
- Documents. Keep the purpose, version, file and signature date together. The expiry date and Pending, Signed, Withdrawn or Expired state help the team decide which document needs attention during the next contact.
What to agree during setup
Agree which evidence supports each consent decision and who records a withdrawal. The Clinical Coordinator and Licensed Clinician can maintain donor documents, while the Clinical Reviewer examines them. The Clinical Governance Administrator uses the donor identity when assigning care access. Each person needs a shared understanding of what a document purpose means and when a replacement version is appropriate.
Choose a starting donor list with enough detail to distinguish people reliably. Review ambiguous names before scheduling collections. Establish a practical document naming convention so staff can locate the signed file during a busy session. The customer’s clinical lead owns the consent procedure and the evidence required for a particular collection.
Check the handoff on an actual case
Use a rehearsal donor called Alex Morgan whose consent is Pending and whose consent document is awaiting signature. The Clinical Coordinator confirms the identity and screening authority, then records the outcome of the consent conversation. When the document is signed, the coordinator records its version and signature date and updates the donor’s current consent position.
Next, rehearse a later withdrawal. Find the same donor and check that the current consent state communicates the change. Open the earlier attachment to understand what was signed and when. The collection owner should be able to explain the current decision from the donor record without treating the older signature as continuing permission. Finish by checking who will handle the next donor contact.
What your team sees
- Donor Consent Register — Find donors by identifying details and compare their current consent position with the named screening authority.
- Pending Donor Documents — Review pending donor attachments by purpose before the next donor conversation.
- Expiring Donor Documents — Find documents whose recorded expiry date has arrived and decide what evidence needs renewal.
Follow the work through
Use donor intake and collection to carry the donor record into a scheduled visit. Use review and follow-up when a document question needs an assigned action.
