What you can do

Explain what still threatens delivery

A defect can expose a wider delivery concern. The correction may depend on scarce expertise, an uncertain technical approach or a delayed supplier response. Project risks capture that concern with an owner, category, probability, impact and mitigation. Project Manager uses the risk record to explain what the team is doing about it and when its assessment changes.

Operation receipts answer a different question: what happened when an action ran? Each receipt can carry the operation type, source reference, occurrence date, actor, result and notes. Successful and failed triage or time-entry operations leave evidence that the team can inspect. A failed attempt should lead to an identified correction, rather than an assumption that the underlying work progressed.

How the work is organized

  • Risk ownership. Record the concern against the project and choose the relevant category, such as Technical, Schedule or Resource. Assign an owner who can explain the mitigation. Keep probability and impact understandable to the Delivery Lead reviewing the wider project position.
  • Assessment changes. An Open concern can move into Monitoring while the team follows its response. Mitigated and Closed describe later assessments that need supporting explanation. A defect becoming Fixed does not automatically resolve a risk whose effect on delivery is still uncertain.
  • Action results. Inspect failed operation receipts by their type and source reference. A missing severity can block defect triage; invalid hours can block time-entry approval. Read the notes and source record together before choosing the corrective action or retrying the request.

What to agree during setup

Agree what Low, Medium and High mean for risk probability and impact. Decide how the team uses the numeric scores and the displayed rating in its review. Establish the calculation and interpretation before relying on a rating to prioritize management attention. The risk owner should be able to explain the underlying assessment in ordinary terms.

Define who checks failed operations and how they find the referenced source record. Agree which notes are needed to distinguish an incomplete request from one that was already processed. Preserve the original receipt when the underlying data is corrected so later reviewers can understand both the failed attempt and the successful result.

Check the handoff on an actual case

The checkout defect needs a specialist who is also committed to another project. Project Manager records a Resource risk with an owner and a mitigation plan. Delivery Lead reviews the active workload and agrees the next staffing action. The defect remains in its own lifecycle while the team monitors whether the staffing decision addresses the wider delivery concern.

At the same time, inspect a failed triage attempt caused by missing severity. QA Engineer corrects the report and checks the later result. In a separate case, a Developer submits zero hours and the time-entry action fails. Project Manager should find that entry through the receipt reference and ask for a valid correction. Neither failure should be confused with a completed operation, and the earlier evidence should remain available after the work is resolved.

What your team sees

  • Active Risks — Open and Monitoring concerns with their owners and mitigation plans.
  • Failed Operations — Unsuccessful delivery actions with source references and explanatory notes.
  • Milestones At Risk — Pending and Missed checkpoints that may need a risk discussion.

Follow the work through

Bring risk and action evidence into project delivery review. Use report and triage or time validation to rehearse failures.
