What you can do
Put professional authority in a record
The Clinical Governance Administrator maintains people, clinician privileges and care-team assignments. A person record connects a staff member or clinician to a user account and notes the person’s active state. A privilege has a type, license number, issuing authority, verification attachment, effective dates and status. Care-team assignments connect a clinician to a patient for an access purpose and effective period.
Those records give the clinic a place to review who should take part in care and on what basis. A privilege marked verified is more useful when its evidence and effective dates are present. An assignment that has ended should not be treated as current merely because the clinician worked with that patient previously.
How the work is organized
- People and accounts. Staff identity is separate from the user sign-in record and can be used consistently in encounter and clinical references.
- Privilege evidence. Verification status, license details and dates let the governance administrator maintain a current professional record.
- Patient assignment. Access purpose and effective dates explain why a clinician is assigned to a patient’s care team.
What to agree during setup
Identify the person responsible for verifying each license and reviewing expiry dates. Decide which privilege types are needed for the clinic’s tasks and what evidence is acceptable. Agree when a suspension is entered and who learns about it. Define the access purposes used in care-team assignments and the process for ending an assignment after a transfer.
The team should rehearse a clinician change on a patient with a scheduled visit. The patient can have an assigned clinician, an encounter has its own clinician and a care-team assignment has an effective period. Agree which of those records the coordinator changes and who checks the privilege before care resumes.
Check the handoff on an actual case
A new clinician joins the clinic. The governance administrator creates the person record, connects the user account and records the clinician’s license, issuing authority and verification evidence. The privilege stays pending until its review is complete. Once verified and effective, the governance administrator can place the clinician on a patient’s care team. The coordinator can then schedule a visit with that person.
If the privilege expires or is suspended later, the administrator updates its status and the coordinator checks affected future visits. The reviewer can inspect the privilege evidence when evaluating a visit. The assignment’s effective dates preserve why the clinician had access during an earlier encounter.
