What you can do

Start with the case

An attachment records its title, evidence type, upload time, visibility, uploader and parent case. Reviewers can follow a file back to the operational case it supports. Internal and Shared visibility let the team make a deliberate access choice for each item.

A file can explain the source of an incident report, a measurement or a later correction. The attachment has its own visibility choice; that choice does not replace the organization boundary on the parent case. An Evidence Clerk should identify the case before uploading, because the parent link is what lets the reviewer find the file beside the decision. The upload time and uploader show when the item entered the case record, while the evidence type tells the next reader why it was kept.

For example, a source document arriving after a case is reported may clarify the affected area. Keep it with that case even if the coordinator later changes the owner. The Independent Reviewer can then compare the file with the source reference on the measured evidence and record a reason if the two disagree.

How the work is organized

  • Source. Identify the organization and service case before entering the next fact.
  • Responsibility. Keep the owner and reviewer visible at each handoff.
  • Result. Use the case status or receipt result to show what has actually happened.

What to agree during setup

Agree which evidence types require a file and who can mark one Shared. Decide whether the file itself or a source reference is authoritative for each review. Keep uploader membership current so the record of who supplied an attachment remains meaningful.

Use a sample Internal file and a sample Shared file to check access with the intended readers. Agree which evidence types require an actual file and which can be traced through a source reference. An attachment cannot itself approve a case revision; the Independent Reviewer records that decision separately. Keep the uploader linked to an active team member so the history still identifies who supplied the material.

Check the handoff on an actual case

Attach a source document to a reported case and set the intended visibility. Ask an independent reviewer to trace it from the case and compare its contents with the linked evidence. If an attachment conflicts with the measurement, return the review for correction and retain the original case link.

On a reported case, have the Evidence Clerk upload the source document and select its visibility. Ask the Independent Reviewer to open the parent case, compare the document with the measured evidence and record a reason if the facts conflict. Repeat with another organization to check that the document is not available outside the case scope. If the case revision changes, review the attachment alongside the newer evidence rather than treating the old decision as current.

What your team sees

  • Record Attachments Register — Show case files with their evidence type, uploader, visibility and upload time.
  • Open Service Cases — Open the parent incident and check its owner and current status.

Follow the work through

Follow case response and evidence review. Use reconciliation when an outcome remains unresolved.
