What you can do

Start with the case

A decision records the reviewer, source revision, decision time, reason and stable operation key. The reviewer should be independent of the person who submitted the evidence. The source and organization must match the case before a decision can guide a later action.

A review decision identifies the reviewer, current source revision, decision time, reason and operation key. Approve, Reject and Return express different outcomes: approval lets eligible work proceed, rejection records why it cannot proceed, and return asks the submitter to correct a specific item. The reviewer must be separate from the person who submitted the evidence or reading.

For a measured event on a case, the Independent Reviewer checks the source, unit, time and organization before recording a decision. If the case revision changes afterwards, the old approval remains history for that revision. It does not establish approval for the revised case.

How the work is organized

  • Source. Identify the organization and service case before entering the next fact.
  • Responsibility. Keep the owner and reviewer visible at each handoff.
  • Result. Use the case status or receipt result to show what has actually happened.

What to agree during setup

Agree who is eligible to review each organization and which case revisions require a fresh decision. The decision choices are Approve, Reject and Return. A returned item should identify what the submitter must correct; a rejected item should preserve the reason.

Name reviewers for each organization and test their independence from the people who submit evidence. Agree when a changed case revision needs another decision and what detail belongs in a return reason. Use stable operation keys so a repeated decision request can be compared with the earlier receipt instead of creating a second effect.

Check the handoff on an actual case

Ask the reviewer to compare evidence with the current case version. Record an Approve decision only after checking source, unit and time. Then change the case revision and confirm the earlier decision is no longer treated as approval for the new version. Inspect the operation key when a request repeats.

Give the Independent Reviewer a source event with a clear unit, time and parent case. Record an approval with the case revision and reason. Change the case revision and show that the earlier decision is history, then record a new decision only after another review. For an ambiguous event, choose Return and explain what the submitter needs to clarify.

What your team sees

  • Review Decisions Register — Read decisions by reviewer, choice, reason and source revision.
  • Service Evidence Register — Compare the decision with the source event and verified time.

Follow the work through

Follow case response and evidence review. Use reconciliation when an outcome remains unresolved.
