What you can do
Specimens and dispensations
A specimen record is tied to an encounter. Staff record its type, collection date and accession number, then track whether it is ordered, collected, received, tested or rejected. This gives the visit team a clear processing state for the specimen associated with a particular patient encounter. Staff use the encounter link to identify the visit behind that accession.
How the work is organized
- Anchor the record. Use the encounter link to identify the visit behind each specimen.
- Capture the evidence. Record accession, collection date and processing status for a specimen.
- Check the handoff. Record medication, lot, quantity, unit and authorizing person for each dispensation.
A pharmacy dispensation also points to the encounter. It records medication, lot number, quantity, unit and the person who authorized it. These details matter when the clinician or reviewer traces what was dispensed for a visit. The encounter link brings the patient and clinician context into view without relying on a free-text patient name.
For a patient with more than one visit, compare the accession number and encounter before changing a specimen state. The reviewer should see which visit supplied the specimen and which visit carried the dispensation.
What to agree during setup
Agree how accession numbers are assigned, who changes a specimen state and how rejected material is handled. Standardize medication names and units for dispensations. Decide who records the authorizing person, and check that a dispensation is attached to the encounter where the medication was provided. If your lab or pharmacy maintains its own operational system, agree the handoff and reconciliation points.
Check the handoff on an actual case
Rehearse a visit where a specimen is ordered and collected. Enter the accession number, record receipt and mark it Tested when that processing state is confirmed. Then record a medication dispensation with its lot, quantity, unit and authorizer on the same encounter. Open the visit and trace both records. Repeat with a rejected specimen so the team can see that it should not be read as tested material.
What your team sees
- Specimens in transit — Find Ordered, Collected and Received specimens that still need attention.
- Visits awaiting review — Inspect the visit before accepting its specialty records for review.
Follow the work through
Follow the connected records in the operating process and agree the decision points before the first live visit. Use implementation to prepare sample cases with the team. Return to the data model when mapping existing patient and encounter records. The team should confirm that each role can reach the records needed for its own part of the handoff.
