What you can do

Preserve the failure that needs a response

A nonconformance carries the failure found during an inspection into the response process. It keeps the inspection link, detection date, severity and description together. The Quality Manager can therefore return to the inspected subject and its check evidence while deciding how to contain the problem and organize the investigation.

Open, Containment, Investigation, Disposition and Closed describe the state of that response. Minor, Major and Critical identify severity. Agree the meaning of those categories before the first operating cycle. A serious failure needs an explicit response owner and a clear handoff; the inspection’s rejected result alone does not explain what the team has done about it.

How the work is organized

  • Failure capture. The Quality Approver rejects failed inspection scope through the review process. The resulting nonconformance remains linked to that inspection, with severity and a description that explain the failed condition.
  • Containment handoff. The review process establishes containment responsibility. The Quality Manager confirms the response and moves the work through Containment and Investigation as the operating team addresses the immediate issue and examines its cause.
  • Linked correction. Corrective actions refer back to the nonconformance. Each action has an owner, due date and stated response. The effectiveness review keeps the failure open until every linked action has been accepted.

What to agree during setup

Define severity in terms the team can apply consistently. A missing label and an unsafe fixture should not depend on the same informal judgment simply because both interrupt work. Choose a clear description practice that identifies the observed condition and its affected inspection scope.

Agree who receives the containment handoff, how they confirm the immediate response, and how the Quality Manager checks it at the daily review. Keep movement or release of physical stock coordinated with the production or inventory system your team uses. Configure that connection separately where it is required. The quality decision needs to remain traceable to the same inspected subject throughout the handoff.

Check the handoff on an actual case

Take a rejected fixture inspection with a damaged locating surface. The Quality Approver raises the linked failure and establishes containment responsibility. The Quality Manager confirms that the affected fixture is handled under the team’s containment procedure, then records the investigation description against the failure.

Create linked corrective work for the immediate repair and for the underlying handling method. Review the repair first while the method change is still awaiting effectiveness review. The nonconformance should remain open. Then accept the remaining corrective work through its review process and verify the final closure behavior. This rehearsal catches a common coordination problem: the first completed action can look like the entire failure has been resolved when another owner still has work outstanding.

During the review, also check that the description remains understandable to someone from the next shift. The inspection reference should lead them to the same physical issue, without relying on the original Inspector being present to explain it.

What your team sees

  • Open Nonconformances — Failures grouped by response stage, with severity and the originating inspection visible.
  • Effectiveness Review Queue — Linked corrective work ready for the evidence decision that governs closure.

Follow the work through

Start with Inspection to disposition, then rehearse Correction to effectiveness across all linked actions.
