What you can do
Preserve the decision trail
A review decision is attached to an engagement and names the reviewer. It stores the source revision, decision, date, reason and operation key. Those details matter when an engagement review is revisited later. The owner can compare the decision with the engagement revision and the proposal or service entry being reviewed instead of relying on the latest status alone. Approval, rejection and return are distinct reviewer decisions; the reason explains what the owner should do next.
Attachments give the team a place for evidence about an engagement. Each attachment has a title, evidence type, upload date, uploader and visibility choice. Internal material stays separate from shared material in the record, and the engagement link keeps the supporting file near the decision. A review should point to the material actually considered rather than a later replacement file with a similar title.
Operation receipts record guarded actions. They store an operation key, expected revision, action, date and result state. A receipt can also hold external and effect references, or a failure reason. Operations Lead can use these records to check whether a requested action completed, failed or needs another review. They are especially useful when an external effect must be reconciled with the engagement record.
How the work is organized
- Decision. Independent Reviewer records approval, rejection or return against a source revision with a reason.
- Evidence. Engagement Owner adds supporting attachments and chooses internal or shared visibility.
- Receipt. Operations Lead reads the result state and external references before treating a guarded action as complete.
What to agree during setup
Define the source revision used for each review request. Decide which evidence types are useful and which materials may be shared. Assign who investigates a failed receipt and who can mark a result complete after checking the persisted effect. Agree on an operation-key convention that connects a repeated attempt to its original action. The date and reason on a decision should be clear enough for someone outside the immediate delivery team to understand it.
Check the handoff on an actual case
For a manufacturing client’s scope extension, Engagement Owner uploads the client’s written request as shared evidence and an internal cost note as internal evidence. Independent Reviewer examines the proposal, records the decision and source revision, and explains the outcome. Operations Lead checks the acceptance operation receipt, its result state and any external reference. A failed result returns to the exception list with its failure reason, rather than being treated as a completed client commitment.
Try a retry of the same action. Compare its operation key and expected revision with the earlier receipt before deciding whether another external effect is appropriate. The engagement history should let an auditor follow the scope, reviewer, evidence and final result in order.
